Below is the list of the basic forms and credentials needed to complete our hiring process. This is not the complete list of forms. Please download, print, complete, and return the documents we have provided -- along with the information and forms that you must provide -- to the address or fax number below. The original forms may not be altered in any way.

Pre-employment Checklist

Information and forms that you provide:

1. Resume
2. Copy of valid Driver’s License
3. Copy of Social Security Card
4. Copy of current licenses
5. Copy of CPR certification (front and back)
6. Copy of any and all current certifications (front and back)
7. TB test or Chest X-ray
8. Immunization/Vaccination Records

Forms that we provide:

2. Letter of Reference (please provide three (3) copies)
3. W-4 (Employee’s Withholding Allowance Certificate)
4. I-9 (Employment Eligibility Verification)
